It’s possible that “my bad” comes from ASL (American Sign Language). It’s a literal translation of two signs that, if more correctly translated mean, “my fault” or “it’s my fault.” Actually, “it’s all good” could also come from ASL but I would guess it has other roots, as it almost sounds like just lazy English, whereas “my bad” is more directly an idiom of sign.
